Qualification for the Arizona Medical Marijuana Program
First, you must have a qualifying medical condition.
If you suffer from any of the conditions below, you could benefit from the use of medical marijuana:
Amyotrophic Lateral Sclerosis
Cachexia or Wasting Syndrome
Severe and Chronic Pain
Severe and Persistent Muscle Spasms
PTSD (Must be in regular counseling or psychiatric care)
While we must adhere to Arizona law, we work to make the process as quick and easy as possible.
Step 1: Medical Record Collection
In most cases, you must have a prior diagnosis with one of the conditions above. This means that you must have a chart note from a healthcare provider proving your diagnosis, and mentioning the treatment that was offered. The state requires doctors to look at records that are from within the last 12 months.
However, if you have a condition that is chronic, or unlikely to go away (like AIDS, obvious physical injuries, cancer, Hepatitis C, etc,) and have not sought medical care in the last 12 months, and for this reason don’t have medical records that are under 12 months old, but you do have older medical records that show proof of the chronic condition, or the condition is VERY obvious (like a severe scar or amputation) we may still be able to certify you. We will require that you have a pre-certification visit, so that the doctor who does the certification will have records from within the last 12 months to review.
In these cases, we can usually schedule a same day pre-certification evaluation. Again, you must have older records with proof of the condition, OR the condition must be obvious to anyone who looks.
If you prefer that we handle your medical record collection, you can complete the medical release form and fax or mail it to our office so that we may request your medical records from your treatment providers. If you already have your medical records, you can get this paperwork to us in any of four ways:

If you sent the records in advance, our physicians will review the records to determine if you qualify. If you qualify for the program, we will call you to schedule an appointment with one of our physicians. If additional documents are required (rare case), we will contact you and let you know which documents are needed for our records.
Step 2: Certification Appointment
At your certification appointment, our physicians will talk with you about the medical condition that qualifies you for the Arizona Medical Marijuana Program. The doctor may talk with you about other treatments aside from medical marijuana. Rarely, the doctor may request more documents if your records are not enough to meet the state requirements. After review and approval, the doctor will produce the documents needed for the state application. These include:
- The Patient Attestation signed and dated by you, stating you will not give medicines to anyone.
- A valid Arizona ID, such as a Arizona ID or Arizona driver’s license, or a US Passport
- A picture of you we will take for for the card
- The Physician’s Certification documentation
- A Caregiver Attestation (if applicable)
- Proof of SNAP benefits (if applicable)
Please note that insurance companies do not cover medical marijuana consultations or the state’s medical marijuana application fee.
After the visit, we take it from there!
Step 3: Application for AZ Medical Marijuana Identification Card

In 1-3 weeks (usually 6-10 business days from the time of the visit) you receive your card from the AZ Department of Health Services in the US mail.
