Are you Qualified?
Basic guidelines to the Certification Process.
First, you must have a qualifying medical condition.
If you suffer from any of the conditions below, you could benefit from the use of medical marijuana:
Amyotrophic Lateral Sclerosis
Cachexia or Wasting Syndrome
Severe and Chronic Pain
Severe and Persistent Muscle Spasms
PTSD (Must be in regular counseling or psychiatric care)
While we must adhere to Arizona law, we work to make the process as quick and easy as possible.
Step 1: Medical Record Collection
In most cases, you must have a prior diagnosis with one of the conditions above. This means that you must have a chart note from a healthcare provider proving your diagnosis, and mentioning the treatment that was offered. The state requires doctors to look at records that are from within the last 12 months.
However, if you have a condition that is chronic, or unlikely to go away (like AIDS, obvious physical injuries, cancer, Hepatitis C, etc,) and have not sought medical care in the last 12 months, and for this reason don’t have medical records that are under 12 months old, but you do have older medical records that show proof of the chronic condition, or the condition is VERY obvious (like a severe scar or amputation) we may still be able to certify you.
If you prefer that we handle your medical record collection, you can complete this medical release form:
Records Release Form
If you already have your medical records, you can get this paperwork to us in any of four ways:
- Upload it in scheduler Scheduler (preferred) OR
- eMail it to: Contact@MMjCCC.com OR
- Fax it to (480) 304-3419 , OR
- Just bring it to your visit with the doctor.
If you sent the records in advance, our physicians will review the records to determine if you qualify. If you qualify for the program, we will call you to schedule an appointment with one of our physicians. If additional documents are required (rare case), we will contact you and let you know which documents are needed for our records.
Step 2: Certification Appointment
At your certification appointment, our physicians will talk with you about the medical condition that qualifies you for the Arizona Medical Marijuana Program. The doctor may talk with you about other treatments aside from medical marijuana. Rarely, the doctor may request more documents if your records are not enough to meet the state requirements. After review and approval, the doctor will produce the documents needed for the state application. These include:
- The Patient Attestation signed and dated by you, stating you will not give medicines to anyone.
- A valid Arizona ID, such as a Arizona ID or Arizona driver’s license, or a US Passport
- A picture of you we will take for for the card
- The Physician’s Certification documentation
- A Caregiver Attestation (if applicable)
- Proof of SNAP benefits (if applicable)
Please note that insurance companies do not cover medical marijuana consultations or the state’s medical marijuana application fee.
After the visit, we take it from there!
Step 3: Application for AZ Medical Marijuana Identification Card
We will handle the application for you!
Or may be sent to: Arizona Department of Health Services 150 N. 18th Avenue Phoenix, AZ 85007
In 1-3 weeks (usually 6-10 business days from the time of the visit) you receive your card from the AZ Department of Health Services in the US mail.
If there are any issues, we will handle them for you. It’s part of our service!
"Like a lot of patients, I was probably a little hesitant to go through the marijuana card certification process, not even certain it would help me in the end. In short, it looked like a big hassle. From my very first inquiry phone call with this office, they couldn't have been more helpful...or professional. It took a bit of effort to gather up my doctors' diagnoses of my back condition and return them to this office for certification, but the actual staff, including the doctors at MMCCC, guided me through the process. The doctor explained the process completely, including the various options and potential benefits if it worked. Can't say enough good things about them. As for my back pain, I've finally found some real relief after over 20 years of living with it."
"I highly recommend this specific office due to the professionalism and the fact that it wasn't obvious what I was there for. MMJCCC always kept me informed on the process and answered every question I had. They legitimately care about your pain and make the whole certification process easy and fast, I got my card within a week of approval and it has greatly helped my pain! Definitely reach out to them for your MMJ card."
"My doctor was very nice and understanding. I brought my paperwork and the process was fairly quick. I had no problem with anything, they walked me step by step and fixed any problem I had. I would absolutely recommend this place."
"Great overall experience. Wasn’t really sure what to expect, but the entire staff makes you feel at ease. Appt was quick and I was in and out in about 45 mins. I would recommend this place to anyone!"
"They had some really nice offices and I didn't even have to wait at all. It was a pleasant experience and I liked how we where able to get things done quickly without much fuzz. Everyone was really nice and professional about the whole thing and I was happy to get my card renewed."
Every one was extremely helpful and answered all my questions. Everything in their literature was spot on and accurate. I would not have been able or willing to do this on my own. Sometimes older folks need a little help too. I got mine respectfully and professionally. I don't write reviews, ever, but I was glad to write this one. 5 stars! Absolutely. (They kept my anonymity something I fear a Yelp review would not do, so this probably will not post, there sorry.)
I do still see doctor's, but they are frustrated cause they know they can't medicate me. They know mmj saved my life and it is no thanks to them. Call me cocky but I love standing tall in front of them and showing them how well I am today because mmj saved my life. I no longer need my wheel chair, I have a full-time job now, I am no longer in a coma state, I am living and enjoying life like I should've been doing 16 yrs ago when those doctor's put me on their plan to death. They almost got me too. Mmj came out just in time to save my life. I lost everything detoxing from their crap, including my marriage and my family. My family is starting to come back, but the man I married and still love even after him abandoning me, smearing me in a divorce, he may never come back. I don't blame him and even though he is cruel I don't believe if I was in his shoe's wouldn't have done the same. Detoxing off narcotics c made me a mess. I never used heroine, but was informed after I detoxed alone on the streets oxycotin was derived from heroine. Now I know why my body went into shock and I was a mean B***h. Thank you and when it's time to renew I will be there.
